Glow Med Lounge Membership Disclaimers and Rules

​


1. Membership Terms & Conditions

  • Memberships are non-transferable and can only be used by the person who is registered. Monthly membership fees are non-refundable and will be charged automatically on each billing cycle on the agreed-upon date. Membership benefits cannot be combined with any other promotions, offers, or discounts unless specified.

 

  • Memberships are on a month-to-month commitment and may be canceled at any time. Services not used in one month will roll over to the next month only if the membership remains active in that second month (exclusions apply). If the membership is canceled, unused services will expire at the end of the billing cycle.

 

  • If a membership is canceled, Glow Med Lounge reserves the right to determine whether it may be reactivated. Reactivation is subject to management approval, payment of any outstanding balances, and agreement to the current membership rates, benefits, and terms in place at the time of reactivation, which may differ from the original agreement.

 

2. Cancellation and Suspension

  • Members may cancel their membership at any time with a 30-day notice. After cancellation, benefits will remain active until the end of the current billing cycle. No refunds will be provided for unused services during the month of cancellation. Memberships may be suspended for non-payment or failure to adhere to membership guidelines.

 

3. Appointment Scheduling & Priority Booking

  • While members enjoy priority booking for services, appointments are still required to access membership benefits. Members must schedule their appointments in advance to ensure availability. Priority booking provides faster access but does not guarantee immediate availability. All services are subject to scheduling, and we encourage members to book ahead to secure their preferred time.

 

  • Rescheduling Notice: If you need to reschedule, please provide at least 24 hours' notice as a courtesy to help us accommodate your new time.

 


4. Services /Perks included with the memberships 

 

a. IV Therapy Membership Perk:

  • All tiered memberships include one IV therapy session per month, allowing members to choose from our variety of IV cocktails (NAD+ therapy excluded). Radiant Glow and Ultimate Glow members receive one complimentary Glutathione add-on per month, which must be administered during the IV therapy session. IV therapy sessions can roll over to the next month only if the membership remains active (Exclusions apply). Memberships are on a month-to-month commitment and may be canceled at any time. If canceled, unused services will expire at the end of the billing cycle.

 

b. Wellness Injection Membership Perk

  • Radiant Glow members receive one wellness injection of choice per month. Ultimate Glow members receive two wellness injections of choice per month. Members can choose from B12, Biotin, Vitamin D3, or Glutathione (200mg). Wellness injections can roll over to the next month only if the membership remains active (Exclusions apply). Memberships are on a month-to-month commitment and may be canceled at any time. If canceled, unused services will expire at the end of the billing cycle. Wellness injections are non-transferable and must be used by the member.

 

c. Chemical Peel Membership Perk:

  • Radiant Glow members receive one complimentary chemical peel every 3 months as part of their membership. Ultimate Glow members can choose between one complimentary chemical peel or microneedling every 3 months. If a member’s membership is not active for a continuous period of 3 months, they will not be eligible to redeem the chemical peel perk until their membership is reactivated and has been active for 3 consecutive months. The chemical peel perk is available only to members who maintain active membership without interruption. This perk is non-transferable and cannot be rolled over or accumulated if not used within the 3-month period.

 

d. Microneedling Membership Perk:

  • Ultimate Glow members are eligible to receive one complimentary microneedling session every 2 months as part of the membership. If a member’s membership is not active for a continuous period of 2 months, they will not be eligible to redeem the microneedling perk until their membership is reactivated and has been active for 2 consecutive months. The microneedling perk is available only to members who maintain active membership without interruption. This perk is non-transferable and cannot be rolled over or accumulated if not used within the 2-month period.

 

e. Birthday Perk:

  • Members are eligible for a birthday perk during their birthday month. This perk is valid only during the member’s birthday month and cannot be rolled over to another month. Any unused birthday perks will expire at the end of the birthday month.

 

5. Membership Consent and Communication

  • Members must sign a membership consent form acknowledging and authorizing the monthly charge processed through our merchant partner, Square, on behalf of Glow Med Lounge LLC. Members agree to receive emails and text messages regarding exclusive member specials, private events, and important membership updates. These communications ensure members stay informed about their perks and upcoming opportunities.


6. Changes to Membership

  • Glow Med Lounge reserves the right to modify, adjust, or discontinue any membership benefits at any time. In the event of changes to membership pricing and/or details, members will be notified with a 30-day notice prior to the new terms taking effect.

 

7. Concierge Service 

  • Glow Med Lounge offers concierge services for all members and patients for a standard fee of $25.00. Ultimate Glow members enjoy a discounted concierge fee of $20.00 as part of their membership benefits. Concierge services are available exclusively for Ultimate Glow members at the discounted rate and are subject to location and availability.

 

8. Exclusive Member Events

  • Exclusive offers and discounts are available during special member events. These events may be limited in duration or availability.

 

9. Share the Glow Referral Program

  • Members will receive their Glow Med Lounge digital gift card once a person from their referral network (friends, family, etc.) has completed a service with Glow Med Lounge LLC. The gift card will be available for use in the same billing cycle their referral completed their service. Gift cards are non-transferable and can only be used by the member who received it. Gift cards cannot be used toward the monthly membership fee.

 

10. Patient Safety & Health

  • At Glow Med Lounge, patient safety is our top priority. All treatments and services will be provided only if the patient is deemed medically eligible and there are no absolute contraindications. Before any treatment, a thorough consultation and evaluation will be conducted to ensure that the service is safe and appropriate for the member. Glow Med Lounge reserves the right to refuse or modify any treatment plan based on health and safety considerations.

 

11. Exclusion Criteria

  • NAD⁺ infusion is excluded from the monthly IV therapy benefit. Members may use their service discount to purchase NAD⁺ infusions separately.


The discounts allotted in each membership tier do not apply to:

  • Neurotoxin (“tox”) treatments

  • Lab draws and analysis

  • Any retail products sold by Glow Med Lounge LLC

  • Las membresías son intransferibles y solo pueden ser utilizadas por la persona registrada.   • Las membresías tienen un compromiso mensual y pueden cancelarse en cualquier momento. Los servicios no utilizados en un mes se transferirán al siguiente mes solo si la membresía permanece activa en ese segundo mes (se aplican exclusiones). Si la membresía se cancela, los servicios no utilizados expirarán al final del ciclo de facturación.

 

  • Si una membresía se cancela, Glow Med Lounge se reserva el derecho de determinar si puede ser reactivada. La reactivación está sujeta a la aprobación de la administración, el pago de cualquier saldo pendiente y el acuerdo con las tarifas, beneficios y términos de membresía vigentes al momento de la reactivación, que pueden diferir del acuerdo original.

 

2. Cancelación y Suspensión

  • Los miembros pueden cancelar su membresía en cualquier momento con un aviso de 30 días. Después de la cancelación, los beneficios permanecerán activos hasta el final del ciclo de facturación actual. No se proporcionarán reembolsos por servicios no utilizados durante el mes de cancelación. Las membresías pueden suspenderse por falta de pago o incumplimiento de las pautas de membresía.

 

3. Programación de Citas y Reserva Prioritaria

  • Aunque los miembros disfrutan de reserva prioritaria para los servicios, se requieren citas para acceder a los beneficios de la membresía. Los miembros deben programar sus citas con anticipación para garantizar disponibilidad. La reserva prioritaria brinda acceso más rápido, pero no garantiza disponibilidad inmediata. Todos los servicios están sujetos a programación y se recomienda a los miembros reservar con anticipación para asegurar su horario preferido.

 

  • Aviso de Reprogramación: Si necesita reprogramar, proporcione al menos 24 horas de aviso como cortesía para ayudarnos a acomodar su nuevo horario.

 

4. Servicios/Beneficios Incluidos con las Membresías

 

a. Beneficio de Membresía en Terapia IV:

  • Todas las membresías por niveles incluyen una sesión de terapia IV por mes, permitiendo a los miembros elegir entre nuestra variedad de cócteles IV (terapia NAD+ excluida). Los miembros de Radiant Glow y Ultimate Glow reciben un complemento gratuito de Glutatión por mes, que debe administrarse durante la sesión de terapia IV. b. Beneficio de Membresía en Inyecciones de Bienestar

  • Los miembros de Radiant Glow reciben una inyección de bienestar de su elección por mes. Los miembros de Ultimate Glow reciben dos inyecciones de bienestar de su elección por mes. Los miembros pueden elegir entre B12, Biotina, Vitamina D3 o Glutatión (200 mg). Las inyecciones de bienestar pueden transferirse al siguiente mes solo si la membresía permanece activa (se aplican exclusiones). Las membresías tienen un compromiso mensual y pueden cancelarse en cualquier momento. Si se cancela, los servicios no utilizados expirarán al final del ciclo de facturación. Las inyecciones de bienestar son intransferibles y deben ser utilizadas por el miembro.

 

c. Beneficio de Membresía en Peeling Químico:

  • Los miembros de Radiant Glow reciben un peeling químico gratuito cada 3 meses como parte de su membresía. Los miembros de Ultimate Glow pueden elegir entre un peeling químico gratuito o microneedling cada 3 meses. Si la membresía de un miembro no está activa durante un período continuo de 3 meses, no será elegible para canjear el beneficio de peeling químico hasta que su membresía sea reactivada y haya estado activa durante 3 meses consecutivos. El beneficio de peeling químico está disponible solo para miembros que mantengan una membresía activa sin interrupciones. Este beneficio es intransferible y no se puede acumular si no se utiliza dentro del período de 3 meses.

 

d. Beneficio de Membresía en Microneedling:

  • Los miembros de Ultimate Glow son elegibles para recibir una sesión gratuita de microneedling cada 2 meses como parte de la membresía. Si la membresía de un miembro no está activa durante un período continuo de 2 meses, no será elegible para canjear el beneficio de microneedling hasta que su membresía sea reactivada y haya estado activa durante 2 meses consecutivos. El beneficio de microneedling está disponible solo para miembros que mantengan una membresía activa sin interrupciones. Este beneficio es intransferible y no se puede acumular si no se utiliza dentro del período de 2 meses.

 

e. Beneficio de Cumpleaños:

  • Los miembros son elegibles para un beneficio de cumpleaños durante el mes de su cumpleaños. Este beneficio es válido solo durante el mes de cumpleaños del miembro y no se puede transferir a otro mes. Cualquier beneficio de cumpleaños no utilizado expirará al final del mes de cumpleaños.

 

5. Consentimiento y Comunicación de Membresía

  • Los miembros deben firmar un formulario de consentimiento de membresía reconociendo y autorizando el cargo mensual procesado a través de nuestro socio comercial, Square, en nombre de Glow Med Lounge LLC. Los miembros aceptan recibir correos electrónicos y mensajes de texto sobre ofertas exclusivas, eventos privados y actualizaciones importantes de membresía. Estas comunicaciones aseguran que los miembros se mantengan informados sobre sus beneficios y oportunidades próximas.

 

6. Cambios en la Membresía

  • Glow Med Lounge se reserva el derecho de modificar, ajustar o descontinuar cualquier beneficio de membresía en cualquier momento. En caso de cambios en los precios o detalles de membresía, se notificará a los miembros con un aviso de 30 días antes de que entren en vigor los nuevos términos.

 

7. Servicio de Concierge

  • Glow Med Lounge ofrece servicios de concierge para todos los miembros y pacientes por una tarifa estándar de $25.00. Los miembros de Ultimate Glow disfrutan de una tarifa de concierge con descuento de $20.00 como parte de sus beneficios de membresía. Los servicios de concierge están disponibles exclusivamente para miembros de Ultimate Glow a la tarifa con descuento y están sujetos a ubicación y disponibilidad.

 

8. Eventos Exclusivos para Miembros

  • Ofertas y descuentos exclusivos están disponibles durante eventos especiales para miembros. Estos eventos pueden ser limitados en duración o disponibilidad.

 

9. Programa de Referidos “Share the Glow”

  • Los miembros recibirán su tarjeta de regalo digital de Glow Med Lounge una vez que una persona de su red de referidos (amigos, familiares, etc.) haya completado un servicio con Glow Med Lounge LLC. La tarjeta de regalo estará disponible para su uso en el mismo ciclo de facturación en que su referido completó el servicio. Las tarjetas de regalo son intransferibles y solo pueden ser utilizadas por el miembro que la recibió. Las tarjetas de regalo no se pueden usar para pagar la tarifa mensual de membresía.

 

10. Seguridad y Salud del Paciente

  • En Glow Med Lounge, la seguridad del paciente es nuestra máxima prioridad. Todos los tratamientos y servicios se proporcionarán solo si el paciente es considerado médicamente elegible y no existen contraindicaciones absolutas. Antes de cualquier tratamiento, se realizará una consulta y evaluación exhaustiva para garantizar que el servicio sea seguro y apropiado para el miembro. Glow Med Lounge se reserva el derecho de rechazar o modificar cualquier plan de tratamiento basado en consideraciones de salud y seguridad.

 

11. Criterios de Exclusión

  • La infusión de NAD⁺ está excluida del beneficio mensual de terapia IV. Los miembros pueden usar su descuento de servicio para comprar infusiones de NAD⁺ por separado.

 

Los descuentos asignados en cada nivel de membresía no se aplican a:

  • Tratamientos con neurotoxinas (“tox”)

  • Extracciones y análisis de laboratorio

  • Cualquier producto minorista vendido por Glow Med Lounge LLC
